Notice from USPTO regarding missing details

I've most recently filed a provisional via the USPTO e-service. I have no received in the mail (paper) a notice for a missing address details. The require a cover sheet with the missing item and an additional $25 fee. They've supplied me with an address.
Is there a way to perform this action online (note that I'm not a registered user on their system).
If sending by mail: Where can I get a cover sheet to print and fill-out? How do I supply the payment? Cash in the envelope? How can I make sure, after mailing, that they've received it and that this was settled?
Since I'm not a US resident, it's hard to take care of this procedure from here, so I appreciate your help in the matter. Thanks.

I'm assuming it is the usual cover sheet that gets sent in with a provisional. Go to this page: http://www.uspto.gov/web/forms/index.html#patentand do a search for "provisional" to get to the latest form SB16. Different versions for paper vs. EFS. Don't send cash! I don't know how you'd get a USPS money order outside the U.S. (any U.S. military bases with an APO/FPO near you?), and a check drawn on an overseas bank might take months (and cost more in fees). A credit card is probably the safest bet, although your card issuer may hammer you with fees for the transaction. As for receipt, if you go with a paper version, you can try including in the envelope a self-addressed stamped postcard, listing the form on it. (In the old days, since carrier pigeons would make a mess on the mailed applications if stuffed in the envelope, postcards were the usual way of doing things.) I'm willing to bet that you can do it via EFS even without a customer number (i.e., being a registered user); call inventor's assistance or the E-business folks to get guidance. EFS should give you a confirmation upon submitting the form.
