Patent assignment

I have patent pending status on an invention. Patent filed 2006. I would like to assign the I P to a LLC owned by myself and family to hold the patent then license it back to my company in the state of Florida. Which form is the right one to down load from your site. There are three assigments products. Which is the one that will fit my needs

I do not recommend doing it yourself. This seems to be a relatively simple transaction and should not be expensive (either on hourly basis or as a fixed fee transaction).
