Patent certificates of correction

Can a patent have multiple certificates of correction? I am a law student with an assignment to draft a patent infringement complaint (we were given an actual real patent number) and I must determine if there is a certificate of correction that needs to be listed in the complaint. The USPTO's "Certificate of Correction File" lists two certificates of correction (2003 and 2004, respectively) but the image of the patent only displays one (the 2003 certificate). Does the patent have one or two certificates of correction, and do I need to list both in the complaint, or only the 2003 certificate?
